RISE – Los Angeles Reentry Programs

RISE is St. John’s Community Health’s reentry program and it is here to help anyone who has had contact with the justice system.

Peer Community Health Workers (PCHW) are here to assist with the difficulties of returning to a free community. St. John’s is committed to providing excellent service to its community in Los Angeles.

Los Angeles Reentry Program Services:

  • CA Identification card and birth certificate vouchers

  • Benefit Enrollment (Medi-Cal, DPSS, SSI)

  • Free legal clinic

  • Employment Preparation

  • Intensive Case Management

Medical Services:

  • Screening for chronic medical conditions (asthma, diabetes, high blood pressure)

  • COVID Testing

  • Health insurance enrollment

Dental Services:

  • Comprehensive dental exams

  • Periodontal therapy (deep cleanings)

Behavioral Health Services:

  • Crisis intervention counseling

  • Group therapy

  • Individual counseling

  • Recovery Treatment

HIV Prevention and Care Services:

  • HIV Testing

  • Medical Care

  • Support Services

  • AIDS Drug Assistance Program (ADAP)

Transgender Health Services:

  • Referrals to transgender advocates, legal support, and other services including assistance with name and/or gender marker changes

  • Trans-specific services, including hormone replacement therapy and referrals for gender confirming surgeries
